Additionally, unlike evaluation codes, therapeutic procedure CPT codes are time-based, meaning you’re billing based on how much time you spend on each service.

Understanding the 8-Minute Rule

Before we discuss the specific codes, it’s crucial that you understand Medicare’s 8-minute rule, also known as the midpoint rule. It basically means that you’ll need to dedicate at least eight minutes of a service to bill for one unit, then additional units at 15-minute intervals.

What Are the Most Common Therapeutic Procedure Codes?

  • Areas treated: You can bill for one or more body areas during a single session

97140 (Manual Therapy)

Modality Physical Therapy CPT Codes

When providing additional treatments that complement your hands-on work and exercise, use modality CPT codes. However, here’s something to keep in mind: modality codes fall into two categories based on the level of supervision.

There are constant attendance modalities (direct one-on-one patient contact) that are time-based and follow the 8-minute rule. Then there are supervised modalities that are classified as service-based codes, meaning you bill for them once per session, regardless of how long you apply them. Also, most modality codes can be used for multiple body areas during the same session.

Supervised Modalities

For supervised modalities, you can set up your patients, ensure they’re safe, and then attend to other patients. They’re convenient, as you don’t have to stay with them the entire time.

97010 (Hot or Cold Packs)

  • Supervision: Can be applied by support staff under your supervision

97014 (Electrical Stimulation – Unattended)

  • Supervision: Can be applied by you or your support staff; you don’t have to be present the entire time

97016 (Vasopneumatic Devices)

  • When to use it: For edema reduction in extremities, improving circulation, and lymphedema management
  • Supervision: You can monitor from a distance
  • Documentation: Explain the pressure settings, the limb treated, and the duration of the treatment session

Constant Attendance Modalities

These modalities work differently from supervised ones, as they’re time-based, and you must remain with the patient throughout the entire treatment to provide continuous supervision and manual adjustments as needed. If you step away, you cannot bill the code.

97032 (Electrical Stimulation – Manual)

  • When to use: For muscle re-education when you need to manually adjust intensity or placement during treatment with direct hands-on contact throughout. It may also be applicable for neuromuscular training that requires real-time feedback.
  • Documentation tip: Be sure to note that continuous manual contact was maintained, how the muscle responded, and any adjustments made.

97033 (Ionotophresis)

  • When to use: To deliver anti-inflammatory medication for conditions like plantar fasciitis, tendinitis, or bursitis while maintaining direct supervision throughout

97034 (Contrast Baths)
